Personal Physician & Health Teams

To ensure the most personalized, coordinated and thorough care in our offices, we are divided into primary care teams, each under the direction of a family physician. During regular office hours, the same medical team addresses each patient's needs. The team consists of a family physician, nurse practitioner or physician assistant. When making an appointment, please be sure to indicate the name of the physician, nurse practitioner or physician assistant you wish to see.
