Family Medicine Associates participates in a large number of medical plans and frequently enrolls in new ones. Please ask us. Please bring your insurance ID card to all office visits and notify us of any change in your insurance. If we participate in your plan, we will bill the plan directly and collect from you any remaining portion your insurance company says you owe. If we do not participate in your plan you will be responsible for full payment at the time of service and we will provide you with an itemized receipt (or other information as required) for you to submit to your insurance plan. Please familiarize yourself with the rules of your plan. Sometimes, routine services, such as physical examinations, are not covered. Frequently, prior authorization is required for emergency treatment and referrals. Co-payment amounts may vary. It is your responsibility to know and understand the requirements of your particular plan, but we are happy to help address any questions you might have.
